Program Coordinator- Jason Hyatt Center/AXS

Position Overview:

The Program Coordinator provides direct supervision and oversight of the Social Rehabilitation Department (AXS/Oasis programs), ensuring efficient operations, staff accountability, and adherence to program goals and outcomes. This role involves regular supervision of program personnel, coordination of admissions and discharges, and implementation of evidence-based practices. Additionally, the coordinator offers support to the Jason Hyatt Center, contributing to intake meetings, service linkages, and cross-coverage within Sound Community Services' integrated system


Essential Functions:

  • Provides leadership and daily supervision for the Social Rehabilitation Department (AXS and Oasis programs), ensuring program effectiveness and adherence to agency standards.
  • Oversees program operations including admissions, discharges, staff scheduling, and facility needs.
  • Conducts regular staff supervision, performance evaluations, and implements accountability and training measures.
  • Plans, schedules, and ensures adequate staffing for community-based social rehabilitation events and activities.
  • Coordinates closely with referral sources and community providers to facilitate client placements and service linkages.
  • Supports the Jason Hyatt Center through cross-coverage and collaboration across multidisciplinary teams.
  • Utilizes data for program evaluation, outcome tracking, and continuous improvement initiatives.
  • Participates in agency-wide administrative responsibilities Administrator-on-Call duties.
  • Maintains compliance with DMHAS requirements and contributes to funding opportunities through program development and grant work.
  • Acts as a member of the management team to support broader agency goals and initiatives.

Physical Requirements:

While performing the duties of this job, the employee is regularly required to sit, use hands to finger, handle or feel objects, tools, or controls, and climb stairs. The employee frequently is required to walk, talk and/or hear; Speaking and hearing ability sufficient to communicate effectively by phone or in person at normal volumes; Vision adequate to read correspondence, computer screen, forms, etc. The employee is occasionally required to stand and climb or balance. The employee must occasionally lift and/or move up to 25 lbs. Specific vision abilities required by this position include the ability to adjust focus.

Qualifications:

Associate degree in Human Services or other related field required; minimum of three (3) years of behavioral health or human services related experience required; minimum of one (1) year experience in a supervisory role that includes leading and oversight of administrative tasks and employees required. Knowledge of evidence-based practices within the field of mental health/co-occurring disorders required; must be proficient in the use of Microsoft Office and other web-based systems; excellent written and oral communication skills required; Current, valid driver's license required; must be CPR/First Aid certified or eligible.
